How to Apply for PCSO’s Medical Assistance

The Philippine Charity Sweepstakes Office (PCSO) institutionalized the Individual Medical Assistance Program (IMAP) to provide timely and responsive financial assistance to Filipinos with health-related problems.

Qualified beneficiaries of the program are issued a Guarantee Letter addressed to a health facility such as a hospital or a dialysis center. Through this letter, the PCSO assumes the obligation for a specific amount due as payment for the health-related service that the facility rendered for the program beneficiary.

The amount of assistance differs depending on the service needed by the beneficiary.

Once qualified, beneficiaries may get financial assistance from the IMAP for the following health-care services—confinement; medicines; dialysis; implant; laboratory or diagnostic procedures; radiation therapy; devices;  and minimally or non-invasive procedures; transplant procedures; cardiac procedures; rehabilitative therapy; and provision for surgical supplies.

To apply for the financial assistance under the IMAP, one has to submit a duly accomplished PCSO IMAP Application Form, which may be downloaded at the PCSO website. These forms are also available at the PCSO’s satellite offices, branch offices, and partner hospitals.

Aside from this, both patient and representative should submit a government-issued identification cards such as passport, driver’s license, GSIS UMID, SSS ID, PRC ID, birth certificate, marriage certificate, digitized voter’s ID, PhilHealth ID, senior citizen’s ID, government employee’s office ID, DSWD – 4P’s ID, and student ID.

It is important to note that there are specific requirements for each type of service that needs to be submitted, on top of the previously mentioned general requirements.

Online appointment system

Applicants may also take advantage of the IMAP’s online appointment system to secure a schedule for the processing of documents and for the interview.

On this online system, one has to identify the nature of assistance that he or she needs, as well as his or her preferred medical institution. The applicant also has to declare whether he or she is an old or new applicant and if he or she is a PhilHealth member, non-member, or a dependent.

Afterward, he or she will be taken into another web page that contains a calendar where the applicant may choose from the available dates to book a schedule.

Once a date has been selected, a new page will appear. This page contains an online form that the applicant or his or her representative will need to fill up with the applicant’s personal information.
